Description  The popular LTPAC Forum is back at HIMSS19! The LTPAC Forum is designed to bring together LTPAC (long-term/post-acute care) stakeholders, highlighting the interrelationships between different care settings. This year’s educational sessions focus on longitudinal care and the importance of coordination and engagement along the care continuum.
